The Tanzania Women Research Foundation (TAWREF) has announced a new employment opportunity for the position of Case Manager under the Epidemic Control (EpiC) Project. The Tanzania Women Research Foundation was established and registered as a Tanzanian Non-Governmental Organization in 2010. The organization focuses on supporting the Government of Tanzania in expanding access to healthcare services, education, shelter, and economic opportunities for marginalized populations.
TAWREF works with evidence-based information to design community-centered interventions aimed at transforming the lives of disadvantaged groups, including women, marginalized men, youth, children, and people living with disabilities. The organization operates both within and beyond the Kilimanjaro Region, addressing critical social and health challenges.
Position: Case Manager
The Case Manager will play an important role in implementing HIV prevention programs targeting populations at risk. The role involves coordinating and monitoring the linkage of HIV-positive individuals to Care and Treatment Centers (CTCs) while ensuring proper case management and client follow-up.
The successful candidate will work closely with Community Engagement Officers, Biomedical Officers, and health facilities to ensure effective service delivery and continuity of care.
Key Responsibilities
The Case Manager will be responsible for:
- Liaising with District Community-Based HIV Coordinators (DCBHCOs).
- Coordinating with health facilities and Peer Navigators for enrollment of clients from the community.
- Supporting retention of clients on Antiretroviral Therapy (ART) and providing feedback to health facilities.
- Managing client appointments and ART refills.
- Tracking missed appointments and coordinating follow-up actions.
- Supporting community-based ART services through proper appointment management.
- Tracking lost-to-follow-up clients and facilitating their return to care.
- Providing mentorship and supervision to Peer Navigators.
- Acting as a link between Peer Navigators and health facilities.
- Supervising peer navigation teams and monitoring case management activities.
- Tracking and reporting all completed and pending referrals.
- Conducting daily follow-ups on case management activities with the Biomedical Field Officer.
- Reconciling and compiling data into regional tracking registers and outcome registers weekly.
- Ensuring all escorted or tracked beneficiaries receive outcomes.
- Conducting site visits for at least 70% of working time.
- Collaborating with the Data Manager/M&E Officer to report tracking and outcome data to the MER team.
- Preparing weekly and monthly reports on tracking services, partner notifications, and success stories.
- Participating in CSO and CHMT meetings.
- Documenting best practices for linkage services among Key and Vulnerable Populations (KVP).
- Performing other duties assigned by the line manager.
Qualifications and Experience
Applicants should meet the following requirements:
- Bachelor’s Degree or Diploma in:
- Clinical Management
- Nursing
- Social Work
- Counselling
- Psychology
- Minimum of 3 years of experience in case management for community-based HIV services.
- Experience working with both literate and non-literate audiences using participatory methods.
- Strong ability to handle data with integrity.
- Experience in Key Population (KP) programs is an added advantage.
- Experience supervising a field-based team.
Required Skills
- Strong communication skills in both Kiswahili and English.
- Ability to work effectively in a team environment.
- Basic computer skills, including Microsoft Word and Excel.

- Contract Duration: 3 months (with possibility of extension).
